ADL limitations/deficits, Limitations in school and/or sports participation, Oral motor deficits, Sensory deficits
Functional Impact of Impairments.....
Picky and rigid eating habits which impact his nutrition and health. Tactile hyper and hypo sensitivities which limit his dressing and grooming tasks and cause concern for safety awareness. Negative tantrum like behavioral responses when he is overstimulated and having difficulty modulating sensory inputs including vision and auditory. Would benefit from OT as it relates to education about sensory integration specific to feeding, tactile and self soothing skills.
Education/Goals......
1.Resolve eating issues
2.Tolerate crowds and non preferred tactile inputs
3.Get services at school
1.Family follow through with feeding recommendations (i.e. provide at least one non preferred food at every meal, make meal times a safe environment, encourage Drew to touch all foods even without having to eat them)
2.Follow through with seeking out therapy services in the school setting (i.e. OT focusing on food tolerance,
tactile exposure, increase proprioceptive inputs for improved sensory modulation)

12 sessions 1x/week then reassess
OT therapy through the school system for the following......

Fine motor skills
Sensory processing/modulation skills
